HOW TO CLAIM
General
1. We are unable to guarantee a claim over the telephone but please phone 01296 611602 as soon as possible to tell us that you need to claim. You do not need to contact us before any treatment begins. We will require your policy number when you call so you should have this ready. Please note that calls may be monitored or recorded to assist with training and for quality control purposes.
2. We will send you a claim form. Ask your vet to fill in the claim form as soon as you can and collect any extra documents and supporting information we ask for.
3. Crufts Pet Insurance has the right to request further information either directly from the vet or from you to confirm the validity of the claim at your expense.

5. We will pay your claim if the claim form is correct and complete:
-When we have all the information we need to support the claim.
-When we are sure the claim is valid.
-When any legal action or other action has been settled.
6. If it is more convenient and your vet agrees, we can pay claims directly to your vet, after deductions.
You can tell us to to do this when you make a claim. We will not pay veterinary fees directly to anyone who is not a vet.
Claim Procedure Under Section 1
Veterinary Fees
1. The claim form must be returned within 60 days of the start of the treatment.
2. When you pay your vet for treatment of an injury or illness which is covered by this policy, keep the receipts.
3. When the treatment is over, send your claim form and receipts to us.
4. An interim claim can be submitted.
5. You must tell us if your dog needs more than one treatment for the illness or injury. Any continuation claims must be submitted at least every 60 days.
6. Please remember that claims only apply to treatment your dog receives whilst the cover is valid.
Claim Procedure Under Section 2
If Your Dog Dies From An Accident
1. Send the claim form to us as soon as possible after your dog dies, along with the purchase receipt and pedigree certificate (if applicable).
Claim Procedure Under Sections 3 & 4
If Your Dog Is Lost, Stolen Or Strays And Costs For Trying To Find Your Dog
1. You must notify your local police station within 48 hours of the theft or loss. Ask for our approval before you advertise or try other methods of finding your dog and keep any receipts for your expenditure. Notify us if your dog has not been recovered after 45 days and send in the purchase receipt and pedigree certificate (if applicable) together with any receipts for advertising costs and a covering letter explaining how the loss occurred.
2. If you are claiming for the recovery of a reward we will also need a receipt giving us the full name and address of the person who found your dog and the amount you have paid them.
Claim Procedure Under Section 5
Boarding Kennel Fees If You Have To Go Into Hospital
1. As soon as practicable contact Crufts Pet Insurance if you or a member of your family needs to be hospitalised.
2. Upon discharge from hospital a medical certificate must be obtained, at your own expense, substantiating the need for hospitalisation.
3. Send us the medical certificate, the receipt from the boarding kennels and a covering letter.
Claim Procedure Under Section 6
If You Have To Cancel A Holiday Or Come Home Early / Holiday Cancellation
1. As soon as practicable contact Crufts Pet Insurance if your dog requires emergency life-saving surgery within 7 days of your departure or whilst you are on holiday and this necessitates cancellation or
curtailment of your holiday.
2. Send a covering letter explaining when and why you had to pay each expense and your supporting documentation to Crufts Pet Insurance. This must include receipted bills from the travel company, tour
operator or from other such parties for the expenses that are being claimed detailing the date and charges incurred as a result of the curtailment or cancellation.
Claim Procedure Under Section 7
If Your Dog Injures Someone Or Damages Their Property / Third Party Liability
1. Contact Crufts Pet Insurance immediately and advise them of any possible claim. Crufts Pet Insurance will then give you instructions on what to do with any letter, claim, writ or summons.
2. Do not admit or accept liability, negotiate or make any payment or promise of payment without our written consent.
3. Do not answer letters from people who may claim against you or who are acting for people who may claim against you without our written consent.
4. You are required to provide us with all the information that we may reasonably require.
5. We will have the sole conduct and control of any claim and legal proceeding relating thereto including the right to prosecute in the name of the insured but for our benefit for any claim, damages or liability.
Claim Procedure Under Section 8
Overseas Travel Cover
In the event that your dog requires veterinary treatment whilst temporarily in Eire or a member country of the PETS Travel Scheme, payment of any treatment will be made by you to the vet whilst you are there.
Upon return home you should telephone Crufts Pet Insurance immediately and report the claim. Crufts Pet Insurance will forward you a claim form for completion and return. This form must be returned complete
with all paid veterinary receipts. Settlement of eligible claims will be made to you, after deductions, in sterling at the current rate of exchange, such payments discharging us from all liability connected with such
claim.
8 a) Quarantine Costs
1. You must retain all relevant receipts because we will require you to support your claim with evidence of expenditure.
2. We will ask you to support your claim with documentary evidence that your dog was microchipped prior to your journey with a microchip of ISO Standard 11784 or Annex A to ISO Standard 11785.
8 b) Repeat Tick And Worming Treatment
1. You must retain all relevant receipts because we will require you to support your claim with evidence of expenditure.
2. We will ask you to support your claim with documentary evidence that the initial tick and worming treatment was obtained and that this was done in the time-scale required by the PETS Travel Scheme.
3. We will ask you to support your claim with evidence that the tick and worming treatment was necessary in order to comply with the PETS Travel Scheme.
8 c) Loss Of Pet Passport
1. You must retain all relevant receipts because we will require you to support your claim with evidence of expenditure.
2. We will ask you to support your claim with documentary evidence that the loss of the Pet Passport was reported to the issuing vet within 24 hours of discovery.
8 d) Emergency Expenses Abroad
1. In all cases you must retain all relevant receipts because we will require you to support your claim with evidence of expenditure.
2. We will ask you to support your claim with documentary evidence that your dog required emergency veterinary treatment, or that you reported your dog as missing to the local police station and animal welfare organisations within 48 hours of loss.
3. We will ask you to support your claim with documentary evidence that the loss of the Pet Passport was reported to the issuing vet within 24 hours of discovery.
4. We will ask you to support your claim with documentary evidence that your original departure for the United Kingdom was delayed by your carrier and that the tick and worm treatment was repeated as a result.

9. The cost of dentistry that is not related to an illness or injury including but not limited to cosmetic dentistry, routine cleaning and descaling or treatment for under/over-shot jaws.
10. The cost of any form of dental treatment for dental disease where your Crufts Pet Insurance has not been in force for at least two consecutive years.
11. Fees for any alternative or complementary medicine unless carried out by a qualified vet and approved by us prior to treatment commencing.
12. Any claim as a result of a 'notifiable' disease e.g. Rabies.
13. Any charges in respect of euthanasia except in the case of humane destruction to alleviate incurable and inhumane suffering or any charges in respect of disposal, cremation or burial of your dog.

TERRITORIAL LIMITS
1. Your dog is covered whilst in the United Kingdom, Northern Ireland, Isle of Man and the Channel Islands.
2. In accordance with the European Economic Community regulations, provided that you comply with the PETS Travel Scheme, this insurance also extends to cover your dog under this scheme whilst temporarily
located in any member country of the PETS Travel Scheme (excluding long haul countries as defined by DEFRA) for overseas travel up to a maximum of 3 trips during any 12 month period and a maximum
stay of 30 days during any 12 month period.
3. The exception to this insurance cover is Eire, which is covered up to a maximum stay of 30 days during any 12 month period.
4. All overseas travel cover excludes third party liability cover for your dog and any claim arising within the territorial limits of Eire or member countries of the PETS Travel Scheme.
5. We recommend you check insurance cover requirements for your dog for the specific country of destination with your travel agent before travelling.

8 c) Loss Of Pet Passport
We will pay
Up to a maximum of £250 towards the cost of a replacement Pet Passport should the original Pet Passport become lost during a journey. This includes any quarantine costs incurred as a direct result of the loss of the Pet Passport.
We will not pay
1. Any destruction, loss or theft of the Pet Passport that occurs prior to the start of your journey.
2. Any claim where the loss of the Pet Passport is not reported to the issuing vet within 24 hours of discovery.
