How do I benefit?
To take advantage of this offer visit the Boots Health Club website and sign up to the Health Club. This only takes a few minutes and it's even quicker if you have a Boots Advantage Card.
Within 4 weeks you'll receive a Health Club welcome pack and a voucher for your free eye test will be included in this. Then, all you need to do is visit http://www.boots.com/opticians to locate your nearest participating store and book an appointment, taking the eye test voucher and Advantage Card with you when you go.
Expiry
You will need to check your free eye test voucher for an expiry date as this is specified on an individual basis.
